Report Safety Incidents

You can report accidents, unfortunate events, and other safety-related incidents from their task app. The incidents are added to the daily log and intimated to the project managers. This helps them to keep track of safety-related issues, identify their trends and the behavior of persons involved, assess conditions that caused the incident, assess damages and losses, etc., and devise risk mitigation plans accordingly.

To open the incidents module

  • Login to the Linarc app
  • Select the project
  • Tap 'Module' at the bottom
  • Choose the 'Incidents' tool

The screen displays the list of incident reports.

Using this module, you can:

  • View safety incidents
  • Report safety incidents

To report safety incidents

  • Tap the '+' sign on the top right

  • Enter the details of the incident:
    • Incident date - By default, the date picker shows the current date and time. If you are reporting an incident that occurred in the past, use the date picker/time picker to mention the exact date and time.
    • Caused by- Tap the dorp dwn arrow on the right to choose the entity responsible for the incident. 
      • Material - Caused by hazardous materials
      • People - Caused by human error
      • Site - Caused by site conditions, weather, natural calamities, etc.  
      • Equipment - Caused by failure, breakdown, or malfunctioning of equipment, tools, and machinery
      • Waste - Caused by garbage, trash, refuse, scrap, debris, etc.
  • Severity - Choose the criticality of the injuries and damages
  • Injury- Whether any workers onsite were injured by the incident. Use the toggle switch to indicate.
    • If yes, tap the toggle switch to change the entry
    • Tap the 'Add/Remove' button on the right to add the crew members who were injured during the incident.
  • Treatment - The kind of medical treatment given to injured persons. Indicate whether the injury required only first aid or hospital treatment.
  • Attachments- Add any attachments related to the incident. 
    • Tap the icons at the right to choose the source.

      • Documents - Tap the  icon to add any documents like medical records/ prescriptions from your device storage.
      • Camera - Tap the  icon to take a new photo using your device's camera. Capture the picture to upload.
      • Gallery - Tap the  icon to select the picture you want to use from your device gallery. Browse your device storage and choose the picture to upload.
  • Comments - Enter a description of the incident.
  • Touch the 'Report' button at the top right to submit the report.

View Safety reports

  • Choose the 'Incidents' tool in the module tab of your Linarc app

The screen displays the list of incident reports.

  • Tap the incident to view its details.

  • Tap the filter icon on the top right to find specific incident reports

  • Tap the checkboxes to select or deselect filter criteria
  • Use the tabs on the bottom to filter incidents by this week or month

Once the filter is selected, the results are displayed on the screen

  • Tap 'Cancel' to exit the filter menu.
